PwD (Person with Disability) Certificate

Ehlers Danlos Syndrome (EDS) comes under locomotor disability. The PHYSICAL DISABILITY CERTIFICATE (PwD) can be availed by any EDS remarkable issues person who is citizen of India.

Please read the information as available here and do approach your regional central or state GOVT HOSPITAL. Kindly do apply as earliest as it takes 6 - 8 months to have the PHYSICAL DISABILITY CERTIFICATE (PwD). Please mention your EDS remarkable issues on locomotor types of disability column. Please connect to any nearest GOVT HOSP which gives and recommends PHYSICAL DISABILITY CERTIFICATE (PwD).

How to apply:

1. Kindly collect all medical reports from - NEUROLOGIST, CARDIOLOGIST, ORTHOPAEDIC, GASTROENTEROLOGIST, GENETIST and other DOCTORS / SPECIALIST / HEALTH CARE PROVIDER.

2. This DOCTORS / SPECIALIST / HEALTH CARE PROVIDER must have written in the prescription that you have ESD / HSD. 

3. Submit your PHYSICAL DISABILITY CERTIFICATE (PwD) request application on Government link of UNIQUE DISABILITY ID (UDID) - https://swavlambancard.gov.in/ 

4. After successful submission you will get an acknowledgement number.

5. Kindly visit the regional central or state GOVT HOSPITAL of your State / UT / District after 4-6 weeks as given in UDID website. Please carry your complete medical reports documentation along with genetic report. 

6. On successful submission of your UDID request documentation to the regional central or state GOVT HOSPITAL of your State / UT / District, the HOSPITAL will call you after 3-4 weeks or earlier for evaluation and verification of your EDS remarkable issues.

7. The MEDICAL SUPERINTENDENT (MS) of this GOVT HOSPITAL will recommended - Physical Medicine and Rehabilitation (PM&R), Orthopedic, Cardiology, etc. for your health check up.

8. On the basis of their recommendation from the Department of - Physical Medicine and Rehabilitation (PM&R), Orthopedic, Cardiology, etc, the panel of DOCTORS committee would justify in giving PHYSICAL DISABILITY CERTIFICATE (PwD).  

9. Probably you will get PHYSICAL DISABILITY CERTIFICATE (PwD) under LOCOMOTOR DISABILITY either 40% or 70% as per the PANEL RECOMMENDATION.

10. This PHYSICAL DISABILITY CERTIFICATE (PwD) will be valid for five years only.

11. You will get an electronic  UNIQUE DISABILITY ID (UDID) card alongwith PHYSICAL DISABILITY CERTIFICATE (PwD). 

12. Latter after five years it has to be renewed for having lifetime PHYSICAL DISABILITY CERTIFICATE (PwD).

The PHYSICAL DISABILITY CERTIFICATE (PwD) for EDS remarkable issues people are all GENUINE & NEED based with the challenges the EDS people have in their daily life.
